The MO State Laymen registration form is online here as a fillable pdf file. You may print multiple blank copies for distribution to others or you may fill out and print your own registration. Information entered into the form CANNOT be saved online or on your computer (unless your have software to edit pdf files) so be sure to print it out once completed.
